***DO NOT USE THIS PRF FORM, IT IS STILL UNDER EVALUATION FOR IMPLEMENTATION***
Planning, Design and Construction is responsible for managing requests to renovate existing facilities or spaces on campus, construct new facilities, and add physical structures to existing spaces. If you are ready to authorize work and commit funds to a project, we ask that you complete a Project Request Form (PRF) describing the work you want completed. To authorize completion of the work, approval from your Dean/Director, and your Provost/Vice President is required. These approvals indicate that appropriate funding will be made available and may be used for the project you are requesting. Once approved, Financial Services will validate your anticipated funding source(s) prior to your request being routed to and accepted by Planning, Design and Construction. It is not the responsibility of Planning, Design and Construction to identify or verify available funding in support of your project request.
Estimate Only
In some cases, minimum charges may apply if a project does not move forward after external services are requested. This includes a minimum $400 charge for third-party construction estimates and a minimum $500 charge for architectural or engineering services. A valid funding source is required to cover these potential costs.
PRF Pre-Application Review Meeting
Once your approved project request is received, attendance may be requested for a PRF Pre-Application Review Meeting. Individuals from Planning, Design and Construction, Space Management, University Safety, Networking & Telecommunications and Facilities Management gather to learn more about the goals of your request and provide solutions for executing the work.
Supporting Documents
"A picture is worth a thousand words" meaning that complex and sometimes multiple ideas can be conveyed by a still image. Requestors are encouraged to upload photos as well as any documentation to further support your request.
University Policy
Per G8.06 Contracts for Construction, all construction, reconstruction, improvement, enlargement, alteration, painting and decorating, or major repair to existing facilities of Missouri State University require this form to be completed before work on the project may begin.
